We are seeking a dynamic and dedicated individual to join our team as a Director of Case Management.
This position is an integral part of our hospital's leadership team and is responsible for overseeing the comprehensive coordination of medical and health services for our patients.
The successful candidate will be a Registered Nurse or Social Worker with a robust understanding of hospital operations and a passion for patient advocacy.
As the Director of Case Management, you will have the opportunity to make a significant impact on patient outcomes and the overall efficiency of our healthcare services.
Responsibilities:
Oversee all aspects of the Case Management department, including patient care coordination, discharge planning, and utilization review to ensure the delivery of the highest quality care.
Develop and implement case management programs and policies that align with the hospital's goals and objectives.
Collaborate with physicians, nurses, social workers, and other healthcare professionals to identify patient needs and develop individualized care plans.
Monitor patient progress and adjust care plans as necessary.
Ensure compliance with all federal, state, and local regulations related to case management and patient care.
Participate in the development and implementation of strategies to improve patient outcomes and satisfaction.
Provide leadership, guidance, and support to the case management team, fostering an environment of continuous learning and improvement.
Analyze and report on case management performance metrics, identifying opportunities for improvement and implementing necessary changes.
Qualifications:
Must be a Registered Nurse (RN) or Social Worker.
Minimum of 3 years of experience in case management, including at least one year in a leadership role.
Excellent knowledge of case management principles, healthcare management, and reimbursement.
Familiarity with federal, state, and local healthcare regulations.
Exceptional leadership and team-building skills.
Strong analytical and problem-solving abilities.
Excellent communication and interpersonal skills.
Proficiency in electronic medical records and healthcare IT systems.
Ability to handle multiple priorities in a fast-paced environment.
